NOTICE (pursuant to the Child Protective Service Law 23 Pa. C.S. Section 6344.2, relating to volunteers having contact with children):
BEFORE actively participating in any capacity as (i) an official coach, or (ii) a parent wrestling mat volunteer, ALL of the following items MUST be submitted to the Summit Wrestling Club Board of Directors (via email to summitwrestlingyouthclub@gmail.com):
1) Disclosure Statement Application for Volunteers which can be found by clicking HERE ;2) PA Child Abuse History Clearance form (dated within 60 months) which can be found by clicking HERE ;
3) PA State Police Response for Criminal Record Check (dated within 60 months)which can be found by clicking HERE ; and
4) FBI fingerprint-based record check ONLY IF if you HAVE NOT been a permanent resident of Pennsylvania during the entirety of the previous ten-year period, which can be found by clicking HERE.
